What Can I Report?

Report is the place for anyone in the UConn community to find the best reporting option for an incident (or the threat of an incident) that they have experienced, and to get information about reporting and support.

The following are types of incidents that have reporting options accessible through Report. If you have experienced one of these, you are encouraged to submit a report. If you’re not sure which report is right for you, use the Form Finder.


Academic Integrity (Undergraduate)

Dishonest or unethical academic behavior that includes, but is not limited to, misrepresenting mastery in an academic area (e.g., cheating), failing to properly credit information, research, or ideas to their rightful originators or representing such information, research, or ideas as your own (e.g., plagiarism).

Amorous Relationship Disclosure

This form may be used by faculty, staff or graduate students in positions of authority to disclose an amorous relationship in an employment or academic setting as provided in the Policy Against Discrimination, Harassment and Related Interpersonal Violence.

Bias Related Incidents

An incident that negatively targets, intimidates, or threatens an individual or group due to race, ethnicity, ancestry, national origin, religion, gender, sexual orientation, gender identity or expression, age, physical, mental, and intellectual disabilities, as well as past/present history of mental disorders.

Crime Tips

Share information on a crime that has been committed, is in commission or is being planned.

Discrimination and/or Harassment

Discrimination, discriminatory harassment, sexual or gender-based harassment, sexual assault, sexual exploitation, intimate partner violence, stalking, retaliation, complicity

Hazing

Any activity without reasonable or legitimate educational value expected of someone for the purpose of initiation, admission into, affiliation with, or as a condition for continued membership in a group or organization that humiliates, degrades, or risks emotional, psychological, and/or physical harm, regardless of a person’s willingness to participate.

Maintenance Request

Request service for a building maintenance issue. Any emergent issues should be called into the Facilities Operations Center at (860) 486-3113. If you have a maintenance issue in a residential building at a Regional Campus, please reference the Campus Housing website for property specific forms.

Off-Campus Parties

A gathering of individuals off-campus where there is an adverse impact on the community and the host or participants are UConn students.

Safety Concerns

Concerns that a UConn student, employee, or other community member may be a threat to their own safety or that of others.

Scholarly Misconduct (Graduate School)

Scholarly misconduct is broadly defined as a failure to uphold standards of scholarly integrity in teaching, learning, research, or service.

Sexual and Gender Misconduct

This can include sexual assault, stalking, intimate partner violence, sexual harassment, or sexual exploitation.

Student or Student Organization Behavior

These can include disruptive behavior, alcohol, drugs, and more.

Violation of Policy or Law

Use this form if you are concerned that a member of the UConn community has violated policies and/or the law.

Workplace Concerns

Concerns about an employee’s behavior.
